NDIS Self-Assessment Template: What to Include

The NDIS self-assessment template is not optional and not ornamental. The NDIA reviews it before approving your application. Your auditor uses it as their audit roadmap. Get it wrong, and you'll spend weeks on non-conformities that could have been prevented. Here's what each module requires.

Module 1: Governance and Organisational Management

Module 1 is your organisational backbone. It asks: Do you have a board or management structure? What's your strategic plan? How do you manage conflicts of interest and compliance? How do you recruit and vet staff? The template typically has 15-20 questions. Answer each honestly. If you're a sole trader, you might not have a formal board, but you still need a documented decision-making process. We documented ours as fortnightly leadership reviews with written agendas and action logs. The template asks for evidence - provide board minutes, conflict of interest declarations, training records for your leadership team. If you don't have formal minutes, start now. Auditors expect to see documented governance. This module usually takes 2-3 weeks to complete thoroughly.

Module 2: Service Delivery and Person-Centred Planning

Module 2 focuses on how you actually deliver supports. Do you involve participants in planning? How do you document support plans? What's your dispute resolution process? How do you respond to complaints? Expect 20-25 questions. You'll reference participant files, support agreements, and communication records. We included samples of our individualised support plans, showing how we align supports to participant goals. The template asks how you assess participant satisfaction - we use post-support feedback forms and quarterly check-ins. This module is where you demonstrate your service quality. Auditors spend the most time here during site visits. Invest time in articulating your person-centred approach. This takes 3-4 weeks.

Module 3: Worker Management and Screening

Module 3 is compliance-heavy. It covers worker recruitment, police checks, professional screening, working with children checks (state-dependent), disability worker checks, and referee verification. The questions are detailed. The template asks: How many workers do you employ? How do you screen them? What's your vetting process? Where do you store screening evidence? State variations matter. NSW DCJ clearances, QLD Blue Cards, VIC Working with Children Checks - each state has different requirements. We maintain a worker screening register listing each worker, screening type, expiry date, and who approved them. This module requires the most evidence gathering - one missing police check or referee call can flag a non-conformity. Allocate 4-5 weeks here.

Module 4: Health, Safety, and Risk Management

Module 4 covers your safety systems. Do you have incident logs? What's your risk register? How do you respond to complaints and concerns? What's your emergency and evacuation protocol? Do you have a safeguarding policy? The template typically has 15-18 questions. Expect to document incidents, near-misses, risk assessments, and corrective actions. We maintain an incident log in ShiftCare, categorizing each entry by date, participant, worker, issue type, and resolution. The template asks how you measure safety performance - we track incident frequency, resolution time, and whether recurring issues emerge. This module takes 2-3 weeks. Auditors pay special attention to incident logs - incomplete or defensive documentation raises red flags.

Your Completion Timeline

Completing the full self-assessment honestly takes 4-6 weeks. Don't rush. Your auditor's confidence in your application depends on clear, thorough answers and credible evidence. Assign someone to own each module. Set weekly deadlines. Share drafts with your team to fill in gaps. Once complete, your auditor uses it to tailor their audit plan. A detailed self-assessment actually speeds the audit - the auditor knows you're organized and prepared. This is your quality foundation.
