Stage 2 is the practical assessment phase. The auditor visits your workplace — head office plus a sample of service-delivery sites — over 1-3 days depending on size. Activities include staff interviews, participant interviews (with consent), observation of service delivery, review of live records like progress notes and incident reports, and verification that documented systems are actually being used. Non-conformities raised here must be closed within 90 days for first-time registration to be granted.
