Progress notes are the day-to-day record of every support session. Each note must include: date, time and duration; worker name; participant name; supports delivered; participant response or feedback; any incidents or concerns; signature or e-signature. Notes must be: contemporaneous (written same day where possible), factual (not opinion), participant-led where possible, and securely stored. Module 3 audits review progress notes against service agreements — they are the proof that contracted services were delivered.
