The full answer

NDIS Practice Standard 1.5 requires every registered provider to have a current service agreement for every participant. The agreement must use plain English and cover: services to be delivered, frequency and duration, total price and payment method, both parties' responsibilities, cancellation policy, complaint process, agreement variation and termination terms. Auditors check that every active participant has a current signed agreement and that delivery matches what's documented. Provider Scale ships a compliant service agreement template in the $999 package.
