A care plan (sometimes called a support plan) is the day-to-day operational document for delivering supports to a specific participant. It sits underneath the service agreement and translates the agreement into actionable detail: communication preferences, behaviour support strategies, medical conditions, emergency contacts, manual handling techniques, social interests. Module 3 audits check that care plans are current, signed, and being followed in service delivery.
