The full answer
Plan reassessment is the NDIS's regular check-in on participant progress and needs. Timing: typically every 1-3 years depending on plan type and participant age. Process: 1) NDIA contacts participant/guardian, 2) Support coordinator or access specialist conducts interviews and assessment, 3) Evidence of outcomes and progress is reviewed, 4) New plan is developed with updated goals and funding, 5) Participant approves new plan. Your role: provide progress reports and goal feedback — show concrete data on skill development, behaviour change, community access achievements. Good progress reports significantly influence funding level increases. Support coordinators will ask you questions like 'what goal progress have we made?' — document this rigorously monthly.
