  1. Date, time, duration of the support
  2. Worker name and signature
  3. Participant name
  4. Supports actually delivered (not what was planned)
  5. Participant response or feedback
  6. Any incidents, concerns or observations
  7. Next-session plan or recommendations
  8. Write factually — observations not opinions
  9. Write contemporaneously — same day where possible

Common mistakes to avoid

  • Generic notes that copy-paste each session
  • Subjective opinions instead of factual observations
  • Notes written days or weeks after the support
