  4. Capture baseline at onboarding
  5. Capture progress at each review
  6. Report quarterly to the participant
  7. Report at plan review to support continuation

Common mistakes to avoid

  • No baseline means no progress story
  • Vague goals make outcomes immeasurable
  • Reporting only at plan review (too late to course-correct)
