The steps
- Stabilise the situation and ensure participant safety
- Determine if the incident is reportable (death, serious injury, abuse, sexual misconduct, unauthorised restrictive practice)
- Notify the NDIS Commission within 24 hours via the Provider Portal
- Notify family/guardian as appropriate
- Document the incident in your incident register
- Conduct a full investigation and submit final report within 5 business days
- Review and update relevant policies/training
Common mistakes to avoid
- Taking "is this reportable?" too long to decide
- Notifying late and triggering compliance escalation
- Investigating without root cause analysis
