The steps

  1. Stabilise the situation and ensure participant safety
  2. Determine if the incident is reportable (death, serious injury, abuse, sexual misconduct, unauthorised restrictive practice)
  3. Notify the NDIS Commission within 24 hours via the Provider Portal
  4. Notify family/guardian as appropriate
  5. Document the incident in your incident register
  6. Conduct a full investigation and submit final report within 5 business days
  7. Review and update relevant policies/training

Common mistakes to avoid

  • Taking "is this reportable?" too long to decide
  • Notifying late and triggering compliance escalation
  • Investigating without root cause analysis
