- Define what counts as an incident (reportable and non-reportable)
- Set up an incident register
- Train staff on the difference between incident and reportable incident
- Set up reporting pathway: 24 hours for reportable to NDIS Commission
- Conduct root cause analysis on every incident
- Update relevant policies/training based on RCA findings
- Report trends quarterly
Common mistakes to avoid
- Confusing incident with reportable incident (under-reporting)
- RCA done as a tick-box not a real investigation
- Trends not feeding back into improvement
