The steps

  1. Define what counts as an incident (reportable and non-reportable)
  2. Set up an incident register
  3. Train staff on the difference between incident and reportable incident
  4. Set up reporting pathway: 24 hours for reportable to NDIS Commission
  5. Conduct root cause analysis on every incident
  6. Update relevant policies/training based on RCA findings
  7. Report trends quarterly

Common mistakes to avoid

  • Confusing incident with reportable incident (under-reporting)
  • RCA done as a tick-box not a real investigation
  • Trends not feeding back into improvement
